Adult Foster Care
Our AFC Homes are licensed facilities that provide 24-hour specialized adult foster care for one to six persons that are mentally ill, HIV/Aids and or developmentally disabled. Basic self-care skills are taught and reinforced in the area of dressing, grooming, eating, bathing, toileting, development and gross motor skills, and language development.
Our SIL Homes are committed to transiting mentally ill/ developmentally disabled residents who have successfully completed a structured group home program, demonstrated that he/she can live independently with minimal supports and or who have expressed an interest to obtain independent living skills. All residents must have a willingness to participate in all aspects of the program.
Community Living Support
General CLS Services include but is not limited to; visits conducted by Direct Care Staffing that include monitoring of household safety and compliance, provide educational training related to IPOS goals/ POC and conduct simulation training's to ensure competence. General CLS services are designed to provide structure to individuals through education, stimulation, encouragement, promoting and reminders.
Monitor for household safety and compliance:
Monitor domestic affairs, Develop time management, Provide education regarding medication management, ADL prompts/reminders, Meal planning, Budgeting, Public transportation assistance, Laundry assistance.
We also provide hands on personal care services; such as bathing, dressing, laundry, toileting, light meal preparation and housekeeping.
